| Drug | Plain | With adrenaline |
|---|---|---|
| Lignocaine | 3 mg/kg | 7 mg/kg |

- Dose on lean body weight, since toxicity comes from the peak plasma level
- Doses of different local anaesthetics add up
- Use less in the elderly and in hepatic or cardiac impairment
- Strength in mg/mL is the % × 10 (e.g. 1% = 10 mg/mL)
Local anaesthetic maximum dose
Maximum dose in mg and mL from the drug, strength and weight. Interactive on the website.
Toxicity
- Initial IV bolus of 1.5 mL/kg of 20% lipid emulsion over 1 minute followed by an infusion at 15 mL/kg/h
- Give up to three bolus doses (two more) of lipid (1.5 mL/kg) at 5 minute intervals if:
- Remains haemodynamically unstable
- Adequate circulation deteriorates
- Continue the infusion until the patient is stable, double the rate to 30 mL/kg/h at any time after 5 minutes if:
- Haemodnyamically unstable
- Adequate circulation deteriorates
- Do not exceed a maximum dose of 12 mL/kg
- Give up to three bolus doses (two more) of lipid (1.5 mL/kg) at 5 minute intervals if:
